I have this in config.txt and my output file is in "c:\vDOS\#LPT1.asc"
LPT1="C:\Program Files (x86)\Printfil\Printfil.exe" #LPT1.asc
I like to have a background file. I can setup it at options in printfil, but then I can not move/scale/select pages for the background. So I need to have a "filename".cfg file (https://www.printfil.com/manualen/c6.htm)
I created a "c:\vDOS\#LPT1.CFG" file in the same dir as the "#LPT1.asc" but somehow it is not changing the background. Is this the right place of this CFG file?

No idea, you would have to ask Printfil. Note: Merging a bitmap image into a printout is generally far from optimal. The resolution will be low and not device independent. Printing to PDF will also result in relative large files. See www.vdos.info/faqs.html - Digital printing with stationary (PDF). Jos

Thanks for getting back to me. I'll reach out to printfil.
Using PDF as a background isn't functioning because printfil can't locate a ghostscript installation.

Eventually print to a virtual PDF printer like Bullzip?

I will stick with printfil, because it has the best font available for me. The background image would only be a bonus, but not needed. Thanks for the info about Bullzip
